Are You Interested in becoming a client at NorthPointe Birth Center?

This form is for individuals who would like to establish care with us.

If you are seeking pregnancy care, please note that we are a low-risk facility. Please review the “Am I Eligible” tab to learn more about our risk criteria and eligibility. We are able to accept clients who are 32 weeks pregnant or earlier at the time of establishing care.

Once your form is submitted, a staff member will contact you within 1–2 business days to answer any questions and, if desired, schedule your first appointment.
