Beloit Health System is hiring for a day shift Registered Nurse Case Manager
to join our team at Beloit Regional Hospice.
This Registered Nurse Case Manager be responsible for overseeing the care
of patients with terminal illnesses and ensuring their comfort and dignity
in the end stages of life. In this role, the Registered Nurse Case Manager
will provide medical, emotional, and practical support to patients and
their families. The duties will include, but not be limited to patient
assessment, care planning, symptom management, documentation, and patient advocacy.
This position requires:
Graduate of an accredited Nursing program, ADN required, BSN preferred
Current IL or WI licensing, with the ability to obtain the other upon hire
Must have current Driver’s License
Strong ability to work collaboratively
Hospice experience helpful, not required
