Beloit Regional Hospice is hiring for a Registered Nurse Case Manager. This is a full-time, day shift position, hours from 8am to 4:30pm.

The RN Case Manager will coordinate care and provide resources to assist patients and families within the community through their end-of-life journey. On- call rotation is required.

Required:

• Graduate of accredited nursing program

• ADN required, BSN preferred

• Unrestricted WI or IL License with the ability to get the other upon hire.

• Valid Driver's License
