The RN Case Manager will provide one-on-one patient centered care in collaboration
with an Interdisciplinary group of like-minded care givers. The RN Case
Manager will coordinate care and provide resources to assist patients
and families.
This position is full time and 1st shift.
For this position we require:
Graduate of an accredited program
Current IL or WI licensing, with the ability to obtain the other
ADN required, BSN preferred. If ADN, BSN to be completed within 5 years of hire.
