Location: Beloit Memorial Hospital - Beloit, WI
Pay Range: $26.80 - 40.20/hour
Shift: 1st
Schedule: 8am - 4:30 pm

This position will:
Review new admissions for any special needs or issues that need to be addressed
Assist with patient discharges from a hospital, clinic, or nursing home
setting to return home
Coordinate the care of patients with complex needs that require multiple
services over time
Handle crisis situations by networking with healthcare workers
Help patients understand financial and emotional resources available to them
Required:
Bachelor’s Degree in Social Work
Wisconsin Social Work license
