Beloit Health System at Beloit Regional Hospice is hiring for a Night Shift
RN Case Manager to join our team.
In this position the Night Shift RN Case Manager will play a crucial role
in supporting patients with serious, life-limiting illnesses and their
families. The Night Shift RN Case Manager will respond to after hour calls
from patients, families, and facilities. The Night Shift Case Manager
will coordinate care with multidisciplinary team, provide end of life
symptom management and education, individualized care planning, coordinate
with facilities and contractors, and documentation compliance.
Requires:
ADN required, BSN preferred
Current WI or IL Registered Nurse license, with ability to obtain the other

Hospice or Palliative Care experience is preferred
