Beloit Health System at Beloit Regional Hospice is hiring for a RN- Case Manager to join our team.

In this position the RN Case Manager will play a crucial role in supporting patients with serious, life-limiting illnesses and their families. The RN Case Manager will coordinate care with multidisciplinary team, provide end of life symptom management and education, individualized care planning, coordinate with facilities and contractors, and documentation compliance.

Requires:

ADN required, BSN preferred
Current WI or IL Registered Nurse license, with ability to obtain the other (must apply within 30 days of hire)
Hospice or Palliative Care experience is preferred
