Details:
Sports physicals for students in 7th grade-college are being offered in July & August at various locations.
The cost is $30 (cash or check only day-of or you can PAY NOW with debit/credit +fees)
There are multiple dates/times and locations, so please find the date/time/location that will work best for you.
There will be a FREE give-away and the physical will be signed by a Doctor or Advanced Practice Provider.
Guardian must be present for youth 17 years or younger.
Please arrive 15 minutes prior to appointment time for check in and paperwork. Wear comfortable attire and gym shoes.
*Note: This physical will not work for incoming freshmen in Illinois or provide any immunizations.
