  He was apologetic for his being late as soon after he greeted us. Just a couple minutes before he walked in I received the email with this survey. I told him about it and jokingly asked him what he would like me to say about him when I filled it out. We laughed and he went to say he'd called letting him team know he'd be late leaving the hospital. Again he was very apologetic about no one telling us that we'd have a bit of a wait. After receiving treatment he again sincerely apologized for the wait and not being informed.
