Common Comorbidities

The following disease are commonly seen in hospice-eligible patients, alongside their primary diagnosis. The presence of comorbidities is used to evaluate a patient’s eligibility. Please report all symptoms and diseases when referring a patient to hospice.

Common Comorbidities
  1. COPD
  2. CHF (systolic or diastolic)
  3. Ischemic heart disease
  4. Diabetes
  5. Neurologic disease (CVA, Parkinsons, MS, ALS)
  6. Renal Failure
  7. Liver Disease
  8. Concurrent cancers (not the primary diagnosis)
  9. AIDS/HIV
  10. Dementia of any source
  11. Refractory severe autoimmune disease (Lupus, Rheumoatoid Arthritis, Etc.)
Additional Factors Affecting Prognosis
  1. Treatments failed or discontinued
  2. End stage disease in advanced age
  3. Loss of function from disease process
  4. Major organ complications/failure
  5. Multiple end stage diseases
  6. Sudden onset of catastrophic illness
  7. Require frequent medication or treatment changes for symptom management
