Care Coordinator openings at Beloit Memorial Hospital. These positions offer a competitive

This position will:
Review new admissions for any special needs or issues that need to be addressed
Assist with discharge planning and coordination of transition of care
Provide and help patients understand financial, emotional and social resources
available to them
May work occasional Saturday hours
Requirements include:
Bachelor’s Degree in Social Work
Wisconsin Social Work license
Medical social work experience preferred, but not required
