Beloit Regional Hospice is hiring for a RN Night Shift Case Manager. The RN Case Manager will provide care for hospice patients needs overnight as part of as interdisciplinary team. The RN Case Manager will assist with coordination of care and provide resources to assist patients and families within the community through their end-of-life journey.

This position requires:

• Graduate of an accredited Nursing program, ADN required, BSN preferred

• Current IL or WI licensing, with the ability to obtain the other upon hire

• Must have current Driver’s License

• Strong ability to work collaboratively

• Hospice experience helpful, not required
