Case Manager Registered Nurse- Home Health

Beloit Health System Home Health is hiring for a Registered Nurse Case Manager.

The RN Case Manager will provide one-on-one patient centered care in collaboration with an Interdisciplinary group of like-minded care givers. The RN Case Manager will coordinate care and provide resources to assist patients and families. This nurse will provide skilled care in the patients home setting assisting them to stay independent. They work under the direction of a physician, with the use of experience and autonomy. Home care is team focused for best outcomes for patients. It includes teaching and training, IV infusions, wound management, medication management, point of care testing, and phlebotomy, etc. Some experience is needed, as the nurse will rely on their assessment skills.

Required:

• Graduate of an accredited program

• Current IL or WI licensing, with the ability to obtain the other

• ADN required; BSN preferred
